Academic/Attendance Report

Use this form to report concerns regarding a student’s class attendance or academic performance. Submitted reports are reviewed by the Student Success Office, and an academic success advisor will reach out to the student to offer guidance and support.

Student of Concern/Care Team Referral

Use this form to refer a student to the Care Team. This includes concerns related to student mental health, physical safety, basic needs, medical issues, family hardships, or any disruptive, concerning, or potentially harmful behavior.

Student Excused Absence Request

Use this form to request an official excused absence in accordance with the CC Excused Absences Policy. While students should complete this form themselves whenever possible, a representative may submit it on their behalf if necessary.

Sexual Misconduct/Discrimination Report

Use this form to report a possible incident of harassment, sexual misconduct, or discrimination. These reports are reviewed by the college’s Title IX team and are guided by the college’s nondiscrimination policy.

Hazing Report

Use this form to report a possible incident of hazing. These reports are reviewed by the college’s Title IX team.

Accident/Incident Report

Use this form to report an accident or incident on campus that resulted in injury, bodily harm, and/or damage to college or personal property.

Academic Dishonesty Reporting Form

This form is used by faculty to report incidents of student academic dishonesty.
